Add Forms to Online Scheduler

In this article, you will learn how to assign forms to your patients during the booking process.

Teledentix allows you to assign forms to be filled out during the appointment booking process on your Online Scheduler micro-site. Forms can be assigned before or after a patient has completed booking their appointment. 

• Navigate to the "Select Template" tab of the Online Scheduler creation process. 
• Under the "Form Wizard" section, click the "Assign form" button to select forms for your Online Scheduler. 
  • You are able to toggle between assigning forms before or after booking.
